Enrolling With Girven Road Medical Centre
The cost of primary health care services for New Zealand Citizens is subsidised to varying degrees by the government. In order to access these subsidies we encourage you to enrol with Girven Road Medical Centre. You are still able to attend our practice even if you are not enrolled, however you will be required to pay the full consultation fee in cash or via eftpos prior to being seen. Please note that you can only enrol at one practice at a time.
If you would like to join our practice please print out and complete our ENROLMENT FORM IN FULL and bring the completed form along with your Birth Certificate, Passport or Community Services Card to the medical centre, where our receptionist will enter your details into our computer system. A New Zealand Drivers Licence is not evidence of residency and will not be accepted.
Each adult over the age of 16 years is required to complete their own enrolment form and produce the correct identification evidence for funding legislation.
We have enrolment forms at our practice and welcome you to collect one during our opening hours. If you are a new patient attending your first appointment with your enrolment form, please arrive 10 minutes early so we can enter your details and avoid delays. On arrival please report to reception.

If your preferred doctor is unavailable our receptionist will offer an appointment with an alternative doctor. On arrival please report to reception. If you are experiencing breathlessness, bleeding or chest pains please report to reception immediately or call 111 for the Emergency Services.
Extended Appointments are Required for the Following
Please discuss talk to reception if you think you may require an extended appointment. In some instances you may need to book a separate appointment. There are additional fees for extended appointments.
Cancellations
Consultation appointments are 15 minutes and we ask that you appreciate the importance for the doctor to keep to schedule. If you need to cancel your appointment, please provide at least 4 hours notice so we may offer the space to another patient.
Repeat Prescriptions
We strongly advise you to arrange an appointment 1 to 2 weeks prior when your medication is nearly running out.
